To obtain a copy of your information:

• Complete an Authorization Form (Adobe PDF).
• Send completed form, along with a legible copy of your photo ID, to the facility where you were a patient. Acceptable forms of identification are government-issued ID cards and John C. Lincoln employee ID cards.
• Mail, fax or deliver forms to the facilities listed below.

Records may take up to 72 hours to process. Fees may apply.
